Person Centred Care

Each person that we help is provided with a person centred plan, this is an individual assessment carried out with the person and / or their families. We help you to identify the key areas of your care where you need the most help and look at ways that we can help support you.

We initially realise short term goals that become achievable in the initial stages of your stay with us. These ‘goals’ furthermore form the basis for long term achievements to be recognised and developed.

An example may be that you like football and enjoy watching it; this can mean following a local team with the idea of playing in that team or becoming involved in other ways.

Lime Trees Care Group provides you with a home environment for as long as the individual wishes to live there and the home is able to meet their identified needs

We pride ourselves on offering a high standard of specialised care service for adults, young and old, with learning disabilities, physical disabilities, behaviours that may challenge and brain injury (ABI & TBI).

24 Hour Specialised Support for the individual

Karol has only recently joined us, transitioning from his own home where he was supported by carers who were no longer able to meet his increasing needs. Following a meeting with our team, attended by Karol and his case manager, we were pleased to offer him a place within the Lime Trees Care Group family.

Although still settling in, Karol is gradually becoming more outgoing and is beginning to explore new interests and hobbies. We are delighted to support him on this journey and look forward to seeing his confidence continue to grow.
