Joe was living in a larger home amongst other people; he wanted to live a more independent life and expressed the wish for relationships and work/ employment. Where he lived, it was not possible for Joe to move in this direction.

Joe’s disability means that he needs for 24hour specialised support, not able to be delivered in a Supported Living Home. The backing of a team solely devoted to his care needed to be set up.

A placement was offered to him. A very comprehensive transition period into a more independent home, looking at avenues to support him in work and help him to form relationships in his local community was undertaken. Staff formed relationships with Joe and were provided with training around areas relating specifically to Joe.

After a period of 3 months Joe moved to his new home with his team that had been working with him in his existing home. He now works in landscaping weekly and continues to establish relationships within his local community while gaining support 24hours per day from staff, consisting of external Psychologists, Occupational Health Teams, Care Management & Personal Support Staff. Who all work carefully to ensure that Joe does not regress in any way. Joe will continue to lead a much more fulfilling lifestyle now and states that ‘he has never been happier’.
